  • Provide accurate MeeSeva center details including District and Franchise Code
  • Upload only valid documents.
  • Verify all details before submission.
Note:
  • This Registration Form is not for allotment of New MeeSeva Center
  • Application submitted by persons not having MeeSeva shall be rejected and Fee paid (if any) shall not be refunded
  • Incomplete or incorrect submissions shall be rejected
  • Use only supported file formats for uploads
  • Verify all details before final submission
